Dawn Michelle Cummins, 37, of Ladoga, passed away Monday.
Born Oct. 14, 1974, at Danville, Ill., she was the daughter of Michael Cummins and Pamela Laperle.
Survivors include her mother, Pamela Vars of Grafton, N.Y.; sons Brent Rhoda and Blake Rhoda; a daughter, Candace Rhoda; a brother, Eric Cummins; and a sister, Stephanie White.
Graveside services will be at 11 a.m. Monday, at Oak Hill Cemetery with burial to follow.
