Leslie Ann Delacruz Hawkins, 29, of Lebanon, and formerly of Crawfordsville, died Thursday afternoon, Jan. 1, 2015.
She had worked in daycare.
She was born July 7, 1985, at Dallas, Texas, to Juanita Ortiz.
Survivors include her mother, Juanita Ortiz; children, Sylus Dovel, Deseria Hawkins and Gabriella Breaks; and siblings, Duvie Gianakis, Gienna Jeffers, Crystal Gianakis, Chelsey Shahan and Shelby Landero.
A funeral service will take place at 2 p.m. Tuesday, Jan. 6, 2015, at Hunt & Son Funeral Home, with Pastor Steve Lee officiating. Burial will be at Oak Hill Cemetery North. Friends may call noon to 2 p.m. Tuesday at the funeral home.
